Another thing is that you need to create a Port forwarding of port 80 and 443 on your router to the Nginx Proxy Manager IP so that the traffic is actually getting there. That should be it. The Proxy Manager will issue a certificate for the Domain name and it should work.

The express app is accessible and it was easy to send the challenge files to get certbot to work. Although not the desired solution it worked. However, I will keep the post open for a better answer.
